City Bank Wellness Program
ELIGIBILITY
All employees (both part-time and full-time) and their spouses are eligible to participate
in the Wellness Program on the first day of the month following 60 days of employment.
If a Wellness Program participant is out on leave for any medically related issue (FMLA),
your participation will be in suspense until you have provided a release from your
doctor.
HEALTH CLUB REIMBURSEMENT
Participants are eligible to receive reimbursement for half of your monthly gym fees up to a maximum of $30 per month. City Bank will cover these costs as long as you work out at your gym a minimum of 12 hours per month. You will be required to submit your workout times on your online calendar by checking the box “performed in gym” for each activity performed at your gym. Reimbursements are paid out quarterly.
ANNUAL HEALTH SCREENINGS
Health Screenings are a vital part of the prevention and detection of many health-related
issues. City Bank offers on-site screenings every year (August-October) in all
of the communities that we serve. Screening fees for employees and spouses covered
by bank insurance will be paid 100%. For those who do not carry City Bank insurance
you may elect to pay for your health screening out of your own pocket, but it is
not required to be part of the Wellness Program. Active participants are eligible
for the health screenings. An active participant is one who logs 16
exercise points per month.
WEIGHT-LOSS INCENTIVES
City Bank will pay you to lose weight! To participate in the weight loss payouts
you must be weighed at the time of the health screenings.
The weight loss payouts will be paid out once a year after the annual health
screenings, at $15 per pound, with no cap on the amount that you can be paid.
Spouses who participate in the weight loss pay outs will also be required to be
weighed at the time of the health screening. Their weight
loss payouts will be paid out on a yearly basis as well. In order to qualify for
payouts, you must be within or above the standardized healthy range of BMI (19 to
24.9); City Bank will not pay for any weight lost to participants with a BMI below
the healthy range. BMI is a standard ratio of weight to height which helps to identify
an individual’s healthy weight. For more information, please discuss your individual
numbers with your physician. Weights and BMI calculations will be recorded yearly
during the health screening by the health screening vendor. Please review the Informed
Consent included in the Wellness Program for complete understanding of your confidentiality.
AWARD INCENTIVES
Award incentives are an exciting element of the program that rewards participants
for healthy lifestyles. Awards are earned through points obtained from exercise:
you earn 1 point for every 15 minutes of exercise. Walking, jogging, running, weight
lifting, and cardiovascular exercise classes are a few examples of approved exercise.
If you feel you have limitations for participation, please consult your physician
to modify your qualifying exercise. Make sure to have your online calendar filled
out before the 10th of each month. You may not log more than 16 exercise points
per week. “Extra wellness points” can be earned through CPR certification (15 points),
blood donation (5 points), plasma donation (5 points), and participation in charity walk events (10 points).
Any extra wellness points will not be
counted as part of the maximum allowed per week. CPR certification is good for 2
years and may only be completed every 2 years. Blood donations may only be completed
as specified by the blood bank (approximately every 8 weeks). Paid time off awards are given
as 8 hours for full time employees and 4 hours for part time employees.
Employees are given 30 minutes of work time each day to walk at your bank location
(if approved by your supervisor), ideally before/after work, or during your lunch
hour.
The Wellness Program is an added benefit to all South Plains Financial, Inc. employees,
and extended to the spouses of our employees. The program and all of its details
